Use the overall rating to build a short list, then look at the health-inspection rating and the latest survey's deficiency count, because those reflect what inspectors actually found; staffing hours matter most for long-term residents.
For a short rehab stay after a hospital discharge, ask about therapy schedules and discharge planning as well; the star rating does not measure either directly.
