What is AZ Care Check?
AZ Care Check is the Arizona Department of Health Services' public portal for licensed health care facilities. For an assisted living center or home it shows the license number, owner, manager, licensed capacity and service levels, every inspection with its statement of deficiencies, and recent enforcement actions.
It is the source behind every row in our directory. Search by facility name, city or license number at AZ Care Check.

How do you read an inspection on AZ Care Check?
Each inspection shows its date, its type, such as an annual compliance inspection, a complaint investigation, an initial inspection or a modification, and an opening statement that says either that no deficiencies were found or that the following deficiencies were found, followed by the cited rules.
Across Pima County, AZ Care Check lists 1,048 inspections of active assisted living facilities, 540 of them with deficiencies cited and 587 that included a complaint investigation. A citation is not a closure: most are corrected through a plan of correction, which the record also shows.
What does an enforcement action mean on AZ Care Check?
An enforcement action is a formal step by ADHS beyond a routine citation, such as a civil money penalty, a consent agreement or a notice of intent to revoke; AZ Care Check lists enforcement actions from the last three years with a decision date and status.
149 of Pima County's 310 active assisted living facilities show at least one enforcement action in that window, most of them closed. 2 facilities' inspection records mention a notice of intent to revoke that was resolved by settlement, and 2 mention a civil money penalty. Read the inspection that led to the action before drawing conclusions.
What should you look for first?
Look first at the latest two inspections and whether either cited deficiencies, then at whether complaints led to citations, then at the license term and any recent change of ownership, which resets the history.
One citation for paperwork is not the same as repeated citations for medication errors or supervision. If the statement mentions a resident injury, an elopement or medication administration, ask the manager what changed afterward and get the answer in writing.
How current is the information?
AZ Care Check updates as ADHS posts inspections and actions, usually weeks after an inspection; our directory copies it in bulk and shows the retrieval date on every page, currently October 30, 2026.
Always reopen the live record just before you sign a residency agreement, because a new complaint investigation can appear at any time.
